Dr. Donald M. Taylor, age 64, of Phoenix, Arizona, passed away December 27, 2002.

Services will be held 11:00 a.m. Friday, January 3, 2002 at Historic First Church, 402 W. Monroe Street, Phoenix, AZ.

Contributions may be made to the Taylor Family Fund, 6360 E. Thomas Rd., #218, Scottsdale, Az., 85251 or Hospice of Arizona, 2222 W. Northern Ave., #A-100, Phoenix, Az., 85021.

Dr. Donald M. Taylor, 64, of Phoenix. Passed away December 27, 2002. Dr. Taylor was born in Spokane, Wash., May 21, 1938.

He was a graduate of the University of Idaho, and Baylor University Medical School. He completed his residency in St. Petersburg, Fla. He served 3 years in the Army Command of M.A.S.H. in Amburg, Germany. From 1968 he practiced in Phoenix as Medical Director for Hospice of Arizona along with his private practice.

He is survived by his wife Kathleen, sons Scott and David Taylor, daughter Holly Mitchell, nephew Don Taylor and twelve grandchildren.

Memorial Service will be 11:00 a.m. Friday at the Historic First
Church, 402 W. Monroe St., Phoenix, AZ.
