Annette M. Holm Kovach, 68, died peacefully on Friday, Oct. 31, 2008 in Flagstaff.

She was born Feb. 28, 1940 in Monroe, Utah to Waldo and Ilean Holm. Annette married John Kovach, Nov. 21, 1959 in Monroe, Utah.

Funeral services were held Monday, Nov. 3, 2008, with a visitation one hour prior at the Church of Jesus Christ of Latter-day Saints, Lark Mary Chapel. Interment took place in the Monroe Cemetery, Monroe, Utah.
