The Arizona Republic, Phoenix, Arizona Wednesday, January 8, 2003 Kathleen M. Cohen, 51, of Glendale, Arizona passed away January 2, 2003. She was born in New York City on February 3, 1951, and moved to Arizona in 1986. She is survived by her son, Evan Louis Cohen, her daughter Beth Ann (Thomas) Johannesen, sisters Ann Marie (Peter) Del Grande, Maureen (Stephen) Gaydarik, Patsy (Emilio) Forte, brother, Patrick John Campbell, and grandson, Thomas A. J. Johannesen. A memorial mass will be held Friday, January 10, 2003 at 5:00 p.m. at St. Joachim and St. Anne's Roman Catholic Church, 11625 North 111th Avenue, Sun City, Arizona. A celebration of her life will follow immediately at the Parish Hall. In lieu of flowers, the family requests that contributions be sent to Hospice of the Valley, 1510 E. Flower St, Phoenix, Az., 85014.
