Casey Forbes, 52, of Phoenix, passed away February 3, 2003. 

Memorial Service will be held at 3:00 PM, Friday, February 7, 2003. VA Medical Center Chapel, 650 E. Indian School Road, Phoenix. 

In lieu of flowers donations may be made to the American Heart Association, 7272 Greenville Ave., Dallas, TX, 75231. Arrangements entrusted to Advantage Colonial Chapel.
