Mildred E. Fohrman, age 76, passed away February 7, 2003 in Phoenix, AZ. 

Survived by her son Scott (Jean) Fohrman of Wilmette, IL, daughter Susan (Christopher) Mead of Riverhed, NY, sister Marcia (Sumner) Shafmaster of North Miami Beach, FL, brother Stan (Barbara) Gass of Deerfield, IL and three grandchildren Jessie and Maia Fohrman and Dylan Mead and many loving nieces and nephews. 

She is preceded in death by parents Samuel and Mena Gass and sister Sally Saltzman. 

Funeral service Monday, February 10th at 9:30 AM in Sinai Mortuary Chapel, 4538 N. 16th St., Phoenix, In lieu of flowers contributions are requested to Hospice of the Valley, 1510 E. Flower St., Phoenix, AZ 85014.
