DONCHAK, Andrew Paul

Andrew Paul Donchak, age 91, a resident of Phoenix, Arizona for 41 years, passed away May 22, 2003. He is survived by his sons; Dennis Donchak, of Seattle, WA, and Andrew F. Donchak, of San Clemente, CA. Mr. Donchak is also survived by his five grandchildren, nieces and nephews.

Mr. Donchak is a WWII veteran serving with distinction in the U.S. Army as a Sgt. Mr. Donchak was a loving Husband, Father and Grandfather.

Funeral services were held Saturday, May 24, 2003, St. Frances Cemetery, Phoenix, AZ. Those who wish, may make contributions to: The Society of St. Vincent De Paul, 420 W. Watkins Rd., PO Box 13600, Phoenix, AZ 85002-3600.
