West Valley View, Avondale, Arizona January 5, 2010 Chris Azotea, 46, of Phoenix died Dec. 29, 2009, in Phoenix. Mr. Azotea was born Dec. 25, 1963, in Manchester, N.H., to Bruce and Maxine. He moved to Arizona 13 years ago from Tennessee. He is survived by his wife, Anita; one daughter, Gina Smith; two sons, Michael Peters and C.J. Peters; his mother, Maxine; four sisters, Tammy Goudreau, Donna Hadley, Debbie Bledsoe and Brenda Turcotte; four brothers, Bruce, Mike, Bobby and Jimmy; and four grandchildren.
