Courtesy of Pat Wilson. ----------------- The Arizona Republic, Phoenix, Arizona Monday, April 21, 2003 Dr. Samuel Milton Chapman, age 82 of Mesa died on April 17, 2003. A native of Mangham, LA and a resident of Phoenix since 1964. "Doc" was a graduate of Moorehouse College of Atlanta, Ga. and moved his dental practice to Phoenix from Baton Rouge. He was also a U.S. Army Veteran having served as a medical officer. He was preceded in death by his wife Pauline. Survivors include; children, Gregory, Greta, Frances "Del", and Pamela, one sister; Frankie C. Fields. Six grandchildren and two great-grandchildren. A host of other relatives and friends. The family will receive friends on Tuesday, April 22 from 6p.m.- 8p.m. at Universal Memorial Center, 1100 E. Jefferson St. Services will be held on Wednesday, 10:00 a.m. from the First Institutional Baptist Church, 1141 E. Jefferson St. Interment to follow at National Memorial Cemetery of Arizona.
