Jack Howard Chesley, 78, passed away January 8, 2004 in Scottsdale, AZ. He was born on June 4, 1925 in Hubbard, AZ. He is a WWII veteran. He is survived by his wife, Juanita; 3 children; Boyd, Ned & Amy; brother, Ben, and his sister, Shirley. A visitation was held on Monday, January 12, 2004 at Meldrum Mortuary, 52 N. Macdonald St., Mesa. The funeral service was held on Tuesday, January 13, 2004 at the LDS Church, 2202 N. 74th St., in Scottsdale, with a viewing one hour prior. Donations can be made to Hospice of the Valley, 1510 E. Flower St., Phoenix, AZ 85014. Published in the East Valley Tribune and the Arizona Republic on 1/11/2004.
