FORMILLER, Alice Mae

Alice Mae Formiller, 85, of Phoenix, a homemaker, died Dec. 10, 1991.  

She was born in Chicago.  

Survivors include her daughter, Arleen Cundary; son, Thomas; three grandchildren; and two great-grandchildren.  

Visitation and services at Paradise Chapel Funeral Home, 3934 E. Indian School Rd.
