Jack is survived by four daughters, Cyndi, Jaclyn, Heather and Alyssa; parents, Jack Sr. and Victoria; sister, Gloria Trembley; brothers, Steven and Kenneth; 1 grandchild and fiancee, Cindy Gentle. Funeral Services will be held, 11 AM with visitation from 10-11 AM, Thursday, July 3, 2003 at Hansen Desert Hills Chapel, 6500 E. Bell Rd., Scottsdale.
