Rhys Alexander Farr was born and passed away Sept. 26, 2011 at Good Samaritan Phoenix Children’s Hospital.  
 
Rhys leaves behind his beloved family, parents, Shaun Michael and Melissa Maureen Farr; sister, Emily Caitlin Farr; grandparents, James and Susan Farr and Mark and Kim Muccilli, all of Lake Havasu City.

Graveside services will be Oct. 11, 10 a.m., at Lake Havasu Memorial Gardens. Mark Sylvester will offer words of comfort and inspiration.

Thoughts and condolences may be submitted to the family at www.lietz-frazefuneralhome.com. Arrangements were placed under the care of Lietz-Fraze Funeral Home & Crematory.
