TELLEZ, Francisco Flores, Jr.

Francisco Flores Tellez Jr., age 42, of Phoenix AZ, passed away December 23, 2000. 

Born July 22 1958, he is survived by his wife, Rachel Tellez; daughters: Sylvia and Sara Frances; sisters: Alicia, Maria and Irma; and brothers: Jose, Jesse and Carlos. 

Visitation was at Chapel of the Chimes Mortuary, 7924 N. 59th Ave., Glendale. Services were held at SS. Simon & Jude Cathedral, 6351 N. 27th Ave.
