Agnes (Ag) Bolden Cook, 66, of Prescott, Ariz., died March 8, 1991. 

She was a retired Supervisor of Surgery from Yavapai Medical Center, past president of Arizona Nurses Association District 5, and the recipient of the 1975 Arizona Superior Service Award from the Arizona Chapter of Public Administrators. 

Preceded in death in 1980 by her husband, Robert Cook II, she is survived by her daughter, Julie Cook of Scottsdale, Ariz.; son and daughter-in-law, Robert Cook III and Veronica of Coraopolis, Pa.; sister, Gracie Patterson; and brother, Leonard Bolden Jr., both of Akron; two grandchildren, Robert IV and Michelle. 

A memorial service was held in Prescott on Monday, March 11. The family 
suggests contributions to Yavapai Humane Society, 1625 Sundog Ranch Rd. 
Prescott, Ariz. 86301, or Hospice of Prescott, 111 N. McCormick St., Prescott, Ariz. 86301.
