West Valley View, Avondale, AZ June 08, 2010 Andrew Stuart Cook, 48, of Avondale died from esophageal cancer June 2, 2010, in Goodyear. Mr. Cook was born Nov. 15, 1961, in Phoenix. He was a freight truck driver for Napa Auto Parts for 27 years. He is survived by his wife, Janet; one daughter, Sara Arthur; two sons, Drew and Vince; his parents, Artice and Don; one sister, Terri Riggs; and one brother, Mark. Services will be private. Memorials may be made to Hospice of the Valley, 1510 E. Flower St., Phoenix, AZ 85014; or the American Cancer Society, P.O. Box 22718, Oklahoma City, OK 73123-1718.
