POLAYES, Sylvia

Sylvia Polayes, 88, passed on February 2, 2004. 

She is survived by her husband Milton Polayes. Also by Barbara Robbins; Susan Forkos; Robyn, Eric & Benjamin Stone; Marc, Angie, Shai, Michael & Alyson Forman. 

Graveside service was held February 3, 2004 at Green Acres Cemetery, 401 N. Hayden Rd., Scottsdale, AZ.
