Charles Richard Tysiak, 79, of Lakeside died March 19, 2012, in Phoenix. He was born Sept. 1, 1932, in Chicago, Ill.

Chuck spent countless hours volunteering in many different ways in the communities where he lived and always helped anyone he met in need. Some of the organizations he was involved in were Knights of Columbus (member since 1955), Summit Healthcare Regional Medical Center (volunteer for 10 years), and served St. Mary of the Angels Parish in many ways.

He is survived by his wife of 56 years, Susan Tysiak; seven children: Sandy Nelson, Beth Johnston, Theresa Tysiak, Loretta Tolmachoff, Andrew Tysiak, Richard Tysiak, Philip Tysiak; 15 grandchildren; three brothers; and two sisters.

Services will be held at 11 a.m. Saturday, April 21, 2 at St. Mary of the Angels Church, 1915 S. Penrod Lane in Pinetop.
