Olivia Jacques, 1, of Prescott, Ariz., born Nov. 11, 2006, in Prescott, Ariz., passed away Dec. 1, 2007, in Phoenix, Ariz. at St. Joseph's Hospital.

A memorial service will be 5 p.m. Thursday, Dec. 6, 2007, at Hampton Funeral Home.

The family entrusted the arrangements to the Hampton Funeral Home.
