BORQUEZ, Theresa

Theresa Borquez, age 30, passed away April 7, 2004. 

She was employed at Banner Baywood Hospital, and formerly employed at Valda Rose Medical Center both in Mesa, AZ. 

Survived by her children, Gilbert, Dominic, Christopher, & Eliana; father, George; brothers, Jason & Robert; and sister, Tonya.

Funeral Service Wednesday, April 14, 2004 at Chapel of the Chimes Mortuary, 7924 N. 59th Ave., Glendale, AZ. Interment at Holy Cross Cemetery, Avondale, AZ
