Jeffrey Raymond Janik, 59, passed away in Prescott, Arizona November 6, 2015. He was born October 8, 1956 in Chicago, IL to Raymond Valentine & Irene (Imbiorski) Janik.

He is survived by his wife of 23 years Nancy Janik; daughter Leia Bond; grandsons Matthew, Kaleb and Zander; and sisters Denise Dear, Lisa Wendt and Nichole Gavin.
