KUBAN, Jack

Jack LeRoy Kuban, 53, a teacher at Sullivan School in Phoenix, died April 
8, 1985.

Mr. Kuban, of Phoenix, was a member of the First Southern Baptist Church 
in Phoenix. He was born in Phoenix. 

Survivors include his wife, Marjorie; son, Martin; mother, Nina; and seven 
grandchildren.
 
A memorial service will be 4 p.m. Monday at his church, 3100 W. Camelback 
Road. Memorial contributions may be made to the Sullivan School, Eighth 
Grade Fund, 2 N. 31st Ave., Phoenix, 85009.
